Maple City Nursing and Rehabilitation is currently seeking an RN MDS Coordinator.
$7,500 Sign-On Bonus Duties Include:
The MDS Coordinator is responsible for assuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines.
Included is identifying resident acuity and needs, helping to determine specific care needs and communicating needs / expectations to families and responsible parties.
The MDS covers such areas as the patient's mood, behavior patterns, cognitive ability and nutrition needs.
Information from these assessments helps nursing home caretakers formulate individualized care plans that include support from social services, dietitians, rehab specialists and medical staff.
MDS coordinators then implement and monitor these care plans to ensure their effectiveness.
They must also make sure all strategies comply with Medicare requirements and ethical standards.
Must meet the requirements of the Department of Health.
Requirements:
Current license as a Registered Nurse (RN) in the state of NY.
Minimum of 1 year of MDS experience gained in skilled nursing facility.
Must understand CMI and maximize CMI.
Knowledge of Medicaid and Medicare.
